Transition Services Team (TST) - For Seniors Transferring from Hospital to Community

A team of health professionals that helps patients with post-hospital community care.
The goal of the team is to help senior clients transition from the hospital back into the community and to facilitate access for clients to needed community based services, such as assisted living, residential care and home care team services.

TST acts as a gateway for post-hospital community care by performing centralized intake needs assessments for hospital inpatient adults (19 years and up) eligible for and needing publicly-funded facilities and services upon discharge. TST also assists in supporting the reduction of Alternate Level of Care (ALC) patient days within Vancouver.
