North Shore Diabetes Foot Care Program
Healthcare Providers and Home Health Care
Provided by Vancouver Coastal Health
Provides assessment, education and treatment for clients with diabetes, with focus on the prevention of wounds, foot ulcers and lower-extremity amputations.
Once a treatment plan is completed, clients are discharged with a referral to other health care providers or to public services as needed for follow-up.
Eligibility: Clients with diabetes (no open wounds) are seen by a foot care nurse, occupational therapist and receive education in self-care and home management.
