Assertive Outreach Team (AOT) - Mental Health and Addictions
Addictions / Substance Use and Mental Health - Adult & Senior

Assertive Outreach Team provides time-limited (28 days) intensive, outreach services to residents of Vancouver who are identified as difficult to engage while also having suspected or identified mental health issues along with substance use issues. They help by providing:
- Frequent outreach visits
- Crisis intervention, apprehension, sectioning
- Intensive monitoring and support
- Accompaniment to MHA-related appointments
- Liaising and connecting with other services
- Daily medication admin
- Monitoring of extended leave
This is a partnership between Vancouver Coastal Health and the Vancouver Police Department, and clients are seen by a team made up of one nurse and one VPD officer.
This program targets:
- Individuals with high use of emergency departments (appropriate or inappropriate)
- Individuals who have been flagged by Police as potentially suffering from mental health or substance use issues
- Individuals who pose a potential risk to the community due to chaotic nature of substance use/misuse or mental health symptoms
Referral: Referrals accepted from VPD and ED (SPH and VGH). This service can be accessed with a referral through the Access and Assessment Centre (AAC).
