- Rehabilitation goal-setting
- Basic support (access to activities)
- Rehabilitation readiness assessment (exploration of service options, building confidence)
- Rehabilitation processes (assistance with goal achievement)
- Referral to other services and advocacy
This program serves clients 19+ and can be accessed with a referral through the Access and Assessment Centre (AAC), or by a family physician, psychiatrist or nurse practitioner.
