Physiotherapy Elder Care Outpatient Clinic

A physiotherapy clinic focused on treating frail elderly patients recently discharged from VGH.
The physiotherapy elder care student-led clinic at VGH manages the gap in service for frail elderly patients who have recently been discharged from hospital with multiple co-morbidities and recent functional decline as well as those with a high falls risk. The goal of the clinic is to improve the functional outcomes of these clients in the hope that we will improve their function, reduce falls, improve recovery times and reduce their number of hospitalizations.

Clients must be willing and able to attend a 1 hour initial assessment and will likely be seen 2-3x per week for follow-up by the physiotherapist. Clients attending this clinic must be willing to be treated by student physiotherapists under the direction of a supervising therapist.

The clients who are eligible for assessment and treatment in this clinic are individuals over the age of 70 (clients aged 64-69 years will be assessed on a case by case basis) who live in the VGH catchment of VCH. These clients may be referred by:
  • Physiotherapists as part of discharge planning from VGH hospital units.
  • Physiotherapists from Community Home Care once the client is able to attend a hospital therapy program.
  • Physicians
  • Other health care professionals (i.e. Occupational therapists/Nurses)

This clinic is ideal for those who:
  • Have recently been discharged home from hospital with ongoing issues (i.e. elderly patients with balance issues, poor physical conditioning)
  • Have an increased risk for falls
  • Have complex chronic disease processes involving neurological, musculoskeletal or multiple systems and have experienced a significant functional decline within the last few months.
  • Require rehabilitation /reactivation to remain in their own home or to further assist in their care within a facility
  • Have the cognitive ability to participate in an exercise program. Mild cognitive impairment (MMSE > 18 if score available) is acceptable if client is able to follow simple instructions.
