Community Health Centre (CHC) - Evergreen Community Health Centre

Evergreen Community Health Centre provides services for people of all ages and works with community organizations and health care providers, such as your family doctor, to keep people and communities healthy.
  • Adult Mental Health Teams: Mental Health Teams provide community-based assessment and treatment to adults who are diagnosed with a major mental illness and experiencing significant problems that interfere with their functioning in daily life.
  • Child and Youth Mental Health Teams: The Child and Youth Mental Health Team at Three Bridges Community Health Centre (West End) serves children, youth and their families with serious mental health difficulties and/or social, emotional or behavioural disturbances.
  • Choice in Support for Independent Living (CSIL): Choice in Support for Independent Living (CSIL) is an alternative method for delivering home support for clients who can direct their care.
  • Home Health Dietitians: Home Health Dietitian services range from providing nutrition care to clients recently discharged from hospitals to nutrition counselling for various conditions using individualized diets devised and monitored by a registered dietitian (medical nutrition therapy).
  • Nutrition: Dietitians provide information and support related to food and nutrition via consultations, clinics, and health promotion.
  • Pediatric and Nursing Support Services: Services and care for children with chronic health, medical complexities and special needs.
  • Vision: Public education and referrals to optometry.

  • Partner with community agencies (neighbourhood house, Community Centre, VSB strong starts etc) to support parents and program operators with child development, feeding, sleep, and infant care information.

    Health information, education, and counselling in homes, preschools, and daycares for children aged 0-5 years. Child health clinics: routine infant immunizations, health information, counselling, and assessment of growth and development.

    **Public Health Nurses who work with families of school-aged children and with youth provide the following services:


  • School immunizations - immunize children against vaccine-preventable diseases at the appropriate age and give booster doses as needed.
  • School surveillance - respond to outbreaks of illnesses or disease in schools.
  • General health consultations - provide consultation and referral service for individual students with an identified health issue. Act as support and resource to help students, families, and school staff access appropriate health services.
